Ischaemic (dying) index and middle fingertips due to poor blood supply in scleroderma

The patient is asleep. The hand is placed on a special suppotive table. The skin incisions are marked

The middle finger is opened up. Note the fat under the skin and the tendons running up the middle of the finger

The middle finger nerves and arteries are exposed

The index finger nerves and arteries are exposed. The fine layer on top of the arteries is stripped off to improve the finger blood supply
